These infections, which often spread through contaminated soil and water, pose a significant health risk and are prevalent in low-income Black communities. In this article, we explore the devastating impact of neglected intestinal infections and the urgent need for structural changes to address this issue.

The Intersection of Neglected Intestinal Infections and Poverty:
Historically, intestinal infections have been more prevalent in areas with high poverty rates and environmental factors that favor the spread of infections. In low-income, mostly Black communities, such as the Mississippi Delta and Lowndes County, Alabama, intestinal parasites like hookworm have infected a significant portion of the population. In fact, more than one in three people tested in Lowndes County were found to be infected with hookworm, a parasitic worm that spreads through sewage exposure and can cause severe health issues.

The Health Implications and Disparities:
Intestinal infections, particularly when left untreated or recurrent, can lead to various health complications. In the Mississippi Delta, our research found that a staggering 80% of children with parasitic infections exhibited high levels of intestinal inflammation, which can impair nutrient absorption and stunt growth. Among adults, elevated levels of intestinal inflammation were observed in 73% of individuals, with 45% being infected with Helicobacter pylori, a bacteria known to cause ulcers and cancer.

Neglected Sanitation Infrastructure and Its Impact:
The neglect of wastewater infrastructure in majority Black communities, both urban and rural, has created a hygienic crisis that has been exacerbated by climate change. Outdated sewage systems and inadequate sanitation contribute to the spread of intestinal infections. To break the cycle of infection and poor health, a concentrated nationwide effort to update and maintain sanitation systems is essential. This not only includes improved access to testing and treatment but also addressing the structural issues that perpetuate these infections.

Limited Awareness and Access to Treatment:
One of the challenges in addressing neglected intestinal infections in the United States is the limited awareness of their continued threat. Unlike in lower-income nations where treatment is more accessible and affordable, the cost of medication in the U.S. is often exorbitant due to the lack of federal regulation. This disparity in access to affordable treatment further exacerbates the health inequities faced by vulnerable individuals.

1. Increase Awareness and Advocacy: It is crucial to raise public awareness about neglected intestinal infections and their impact on marginalized communities. Advocacy efforts can help drive policy changes and secure funding for infrastructure improvements.
